COVID-19 Community Vaccine Clinic
With guidance from the Missouri Department of Health & Senior Services and the Center for Disease Control (CDC), COMTREA is a committed partner in administering the COVID vaccine to Jefferson County residents. Prioritization of population eligible will follow the Missouri state pre-determined tier system. For the most up-to-date information, visit your state and local health department websites. Required Vaccination Forms
Download Consent FormDownload Screening FormPrint the Consent Form and Screening Form and bring to your appointment. If you do not have access to print, we will provide paperwork at the door.
confirmation and reminders
Based on the selected communication method listed on your registration, you will receive a text, email, or phone call confirmation of your appointment after we have validated your information.
Arriving Early or late / cancelling
If you arrive early, wait in your vehicle until the time of your appointment. Please be punctual and arrive at your selected time. Please cancel if you cannot make the appointment; if you do not attend, this may waste a vaccine.
Masks and social distancing
Masks are required; the clinic will be setup to ensure social distancing
What to expect When you enter the clinic
Staff will review your consent and screening forms
You will receive the vaccine from a nurse
You will wait 15 or 30 minutes to ensure you do not experience an allergic reaction
Your second vaccine appointment will be scheduled before you leave the clinic

Protecting those who are at increased risk for severe illness
- Anyone 65 and older
- Any adults with:
- Cancer
- Chronic Kidney Disease
- COPD
- Heart Conditions
- Weakened immune system due to organ transplant
- Severe obesity (BMI>40)
- Pregnancy
- Sickle Cell Disease
- Type 2 Diabetes Mellitus
- Individuals with intellectual and/or developmental disabilities such as Down Syndrome
