Patient-Centered Medical Home (PCMH) is a system of care in which a team of health professionals work together to provide all of your health care needs.  We use evidence-based guidelines, technology such as electronic medical records to communicate and coordinate your care and provide the best possible outcomes for you. 

You, the patient, are the most important part of a PCMH where you choose your own primary care provider. When you take an active role in your health and work closely with us; providing a complete medical history, you can be confident in receiving the right care.

PCMH Benefits:

  • Available - Communicate with us easily and efficiently to schedule appointments quickly.
  • Health Education - Understand your conditions and how to take care of yourself. Know your options and make educated decisions about your care.
  • Coordinate Services - Find specialists, schedule appointments, and receive information from your other providers.
  • Technology - Health records made available electronically to efficiently share information with other providers while increasing accuracy and patient-centered communication. 

Your care team will:

  • Learn about you, your family, life situation, and health goals and preferences.
  • Take care of any short-term illness, long-term chronic disease, and your all-around well-being.
  • Keep you up-to-date on vaccines and preventive screenings.
  • Connect you with other members of your care team (specialists, health coaches, etc.) and coordinate your care with them as your health needs change.
  • Be available to you after hours for your urgent needs.
  • Give you information about classes, support groups, or other services that can help you learn more about your condition and stay healthy.
  • Help you make the best decisions for your care.

What you can do:

  • Come to each visit with updates on medications, dietary supplements, remedies, or questions .
  • Communicate when you see other providers, so we can help coordinate the best care for you.
  • Understand your health condition: ask questions about your care and ask for clarification. 
  • Learn about your condition(s) and what you can do to stay as healthy as possible.
  • Follow the plan that we have agreed is best for you.
  • Take medications as prescribed.
  • Contact us after hours if your medical situation  cannot wait until the next business day.
  • If possible, contact us before going to the emergency room, so someone who knows your medical history can care for you.
  • Agree that all health care providers in you care team will receive all information related to your health care.
